Most independent care providers in the UK are a registered manager, a small office team and a workforce of carers on shifts around the clock. Care is delivered in people's own homes or in a home of twenty or thirty beds, paid for by a mix of council contracts and private clients, and inspected by the CQC. The manager is the rota, the on-call phone, the compliance officer and the family liaison, usually all before nine in the morning.

The leaks are the same in every service. The rota is built by hand and rebuilt from scratch when someone calls in sick at six. Care notes are written on paper or typed into a system nobody has time to read, so the picture of how someone is doing lives in individual heads. Families hear from you when something goes wrong, not when it goes right. DBS renewals, training certificates and right-to-work checks sit in a spreadsheet that is accurate until it is not. Invoices to councils and private clients go out late because nobody has had a spare afternoon, and evidence for inspection is gathered in a panic the week before. A home-care agency with thirty carers and a 24-bed residential home have the same problem: the people who could sort the admin are covering the shifts.
